Overview

My research programme focuses on integrating evidence from clinical trials and real-world data to support timely diagnosis, improve disease characterisation, and personalise treatment in chronic airway diseases, particularly COPD. I have particular expertise in interpreting complex clinical research data, helping to ensure that clinical and policy decisions are aligned with what the evidence truly supports.

Biography

Alexander G. Mathioudakis is a Senior Lecturer in Respiratory Medicine at the University of Manchester and an Honorary Consultant Respiratory Physician at Manchester University NHS Foundation Trust. His work focuses on chronic airway diseases, particularly COPD, at the interface of clinical research, evidence synthesis, and guideline-relevant decision-making.

He has published over 165 peer-reviewed articles, with more than 27,000 citations and an h-index of 54 (Google Scholar). His research has informed national and international clinical guidelines and policy documents, and he is currently Principal Investigator of a major NIHR Health Technology Assessment (HTA)–funded programme.

He has contributed to the work of the European Respiratory Society over several years and currently supports its activities as Chair of Group 5.1 (Airway Pharmacology & Treatment), a member of the Advocacy Council and Guidelines Working Group, and Chair of an ERS Task Force developing a Core Outcome Set for COPD maintenance management. He also holds editorial roles in respiratory and evidence-based medicine journals.

Research interests

My research interests focus on improving the diagnosis, characterisation, and treatment of chronic airway diseases, particularly chronic obstructive pulmonary disease (COPD), through the integration of evidence from clinical trials and real-world data.

Key areas of interest include:

  • Treatment response heterogeneity and personalised prescribing in COPD

  • Withdrawal effects and dynamic treatment effects in chronic respiratory disease

  • Interpretation of clinical trial evidence, including estimands, effect modification, and real-world applicability

  • Individual-participant data meta-analysis and evidence synthesis to inform clinical and guideline decision-making

  • Use of routinely collected clinical data to support earlier diagnosis and treatment optimisation in airway disease
