Description
The Salford Integrated Care Programme is a large scale transformation of services in Salford to improve care for older people with long-term conditions and social care needs.
This project (a) provides a rigorous test of the ability of the Salford Integrated Care Programme to improve older people’s experience of care, their well-being and quality of life, and reduce costs of care, and (b) generates generalisable knowledge on the effective delivery of better care for older people with long-term conditions.
