    Abstract

    This article explores the application of reliability tools (FTA and RBD) to analyse the worst inland crude oil leak that resulted from pipeline failure in the United States of America. The primary aim of the reliability tools applied in this article is to identify major contributors to the analysed failure and eventually provide recommendations that might avert future reoccurrence.
