Abstract

Digital ulcers (DUs) are a serious complication of systemic sclerosis (SSc) and can result in significant pain and disability. The purpose of this chapter is to provide the clinician with all the knowledge that is required to successfully manage SSc-DUs. We will explore the relevant pathophysiology, clinical assessment and an approach to the management of SSc-DUs. We will describe how to approach the assessment of pain, disability and functional impact, and local DU complications, including infection. Local (wound bed management), systemic (both to healing and prevention) and surgical treatments will be discussed. The key role for the multidisciplinary team will be evident throughout. Prompt assessment and treatment of SSc-DUs can significantly improve patient’s quality of life and potentially prevent significant ischaemic tissue loss.
