Abstract
A case of a 75-year-old female bearing an epithelioid leiomyosarcoma of the retroperitoneum with granular cell change is reported. The cells were spindle to polygonal with finely granular cytoplasm. The smooth muscle differentiation was demonstrated by the immunohistochemical positivity of the neoplastic cells with anti-α-smooth actin and anti-desmin antisera. The granular cytoplasmic changes are related to an abnormal number of lysosomes within the neoplastic cells as seen by their positivity with anti-KP1 (CD68).
