    Abstract

    This paper examines the causes of a catastrophic failure at the Imperial Sugar Company (ISC) in 2008 that led to the death of 14 workers. Four popular risk assessment tools (failure mode effects analysis, risk priority number, fault tree analysis, and reliability block diagrams) were used to thoroughly analyse the fail-ure, which then led to the generation of several recommendations that may avert such failures in the future.
